La mayor participación atlética en la historia de ODECABE fue en los Juegos en Cartagena de Indias, Colombia en el 2006. De un total de 4865 atletas, 2978 fueron masculinos y 1887 femeninas; incluyendo las sub-sedes de la República Dominicana y México.

La atleta cubana Tania Pérez, quien se especializó en pistolas, es la segunda líder en medallas, obteniendo 11 de oro, 3 de plata y una de bronce para un total de 15, siendo Anita Lallande de Puerto Rico la primera al ganar 17 medallas.
